How Can I Request My Transcript?

For an $10.00 per copy fee, you may order your USF academic transcript online by credit card through OASIS, the University of South Florida's Online Access Student Information System. Login with your student User ID and 6-digit self-assigned PIN which is, essentially, your electronic signature. Under Grades & Transcripts click Request A Transcript to access the online request form.

You may also request $10.00 copies of your USF academic transcript by supplying the Office of the Registrar with your signed authorization. By law, your request must include your signature and date, and you must have no financial obligations to the University. Transcripts are normally mailed/ready for pick-up from the Tampa campus within two working days after your request is received. A USF Transcript Request Form is still available for a limited time (click here to download now) or at the Tampa campus Registrar's Office, the regional campuses' Records & Registration offices, or any campuses' Cashier's offices.

Your request for your USF academic transcript must include:

a.. date of request and your current address
b.. your student ID number and full name
c.. the name and complete address of your intended recipient(s)
d.. fees for the number of copies and special instructions (see below)
e.. your signature

Special instructions:
Explain any special instructions such as "hold for your degree to be posted", "hold for your current term grades to be posted", or "hold for a grade change to be processed". Your degree statement(s) posts approximately five weeks after the commencement ceremony. If you are currently enrolled, term grades are posted approximately one week after all final exams end.

To order your transcript by mail, send your $10.00 per copy payment by check or money order only and your USF Transcript Request Form to:

To order your transcript in person, hand-carry your $10.00 per copy payment (please note all transcripts are produced only on the Tampa campus) and your USF Transcript Request Form to a USF Cashier's Office at any of the following Campus locations:

b.. St. Petersburg - Bayboro Hall, Rm. 102
c.. Sarasota - USS 805A
d.. Polytechnic - LTB 1135

Note: Official transcripts for students who previously attended New College of USF or participated in the M.D. program of the College of Medicine must be requested directly from those colleges:
